Annual File Disclosure Pursuant to the Fair Credit Reporting
Act
To request a copy of your file, you can call our special
toll-free number from 10:00am – 10:00pm Central Standard
Time, Monday – Friday. You will be asked to provide
your current drivers license or state ID number, check routing
and account number, and current mailing address. If we cannot
validate your identity over the telephone from the information
you give us, you may be asked to mail in a copy of your drivers
license or state ID number, a voided check, and a utility
bill to verify that you are entitled to the information that
you are requesting.

Written requests, along with a copy of your current drivers
license or state ID number, a voided check, and a utility
bill should be mailed to:

Your file will be mailed to you within fifteen days from
the date the request was received.
Check Authorization Inquiries
Inquiries concerning a payment attempted when using your
checking account at one of our merchant subscribers, please
contact our Authorization Support Department by phone or mail.
The following information will be required to process your
request:
- First and last name
- Complete mailing address
- Bank routing number
- Checking account number
- State issued driver license or identification card information
- Merchant name
- Date of transaction
- Check number
- Check amount
